Immunization Exemption Form
In the event of a disease outbreak, a child exempted from Idaho childcare immunization requirements may be excluded from a childcare facility for the duration of the outbreak.
Medical Authorization Form
Parent permission is required for Sunscreen, Toothpaste, Bug Spray, Lip Balm, and Non-Medicated Diaper Cream. Anything else must have a signed prescription from the doctor.

ICCP Billing Form
The Idaho Child Care Program (ICCP) serves families with children under the age of 13 or over 13 if a child has a disability, providing child care assistance to working families by paying for a portion of child care. The parent’s share is based on the size of the family and the amount of their income.
